Recognise Nephritic Syndrome

Haematuria (often with red cell casts, dysmorphic RBCs) + proteinuria (sub-nephrotic) + AKI + hypertension + oedema. Rapidly progressive GN (RPGN) = AKI evolving over days–weeks, often with crescents on biopsy — treat as renal emergency. Bloods: U&E, ACR/PCR, FBC, CRP, complement (C3, C4), ANA, ANCA, anti-GBM, anti-dsDNA, anti-PLA2R, immunoglobulins, ASO/anti-DNase B, hepatitis B/C, HIV, cryoglobulins. Urinalysis + sediment.
