Screening + Diagnosis
Risk-based screening at booking + 24–28 weeks if any: BMI ≥30, previous macrosomia (≥4.5 kg), previous GDM, family history first-degree DM, ethnicity (South Asian, Black, Middle Eastern), previous unexplained stillbirth.
Diagnostic OGTT (75 g): GDM if fasting glucose ≥5.6 mmol/L OR 2h ≥7.8 mmol/L (NICE 2015 thresholds).
Glycosuria: ≥2+ on 1 occasion or 1+ on 2 — investigate.
