NICE NG56 — proactive multimorbidity assessment for: • Frail / elderly patients. • Polypharmacy (≥10 regular medications). • Significant treatment burden. • ≥2 long-term conditions adversely affecting daily life. • Hospital admissions in last year. Approach: tailored, holistic, patient-centred — recognise individual patient priorities matter more than disease-specific protocols.
